How and why it works
Clindamycin is a lincosamide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of Cutibacterium acnes (C. acnes). By reducing the C. acnes population within follicles, it decreases the production of bacterial lipases that break down sebum triglycerides into inflammatory free fatty acids. This dual action—antibacterial and anti-inflammatory—makes it particularly effective for inflammatory acne (papules, pustules). Per AAD guidelines, topical antibiotics should be used with benzoyl peroxide to minimize antibiotic resistance risk.
How to use
Apply a thin layer to affected areas twice daily (morning and evening) after cleansing. Can be applied to slightly damp skin. Always combine with benzoyl peroxide (applied at a different time of day or mixed) to reduce antibiotic resistance. Typically prescribed for 12-week courses; avoid long-term monotherapy. Results begin within 2–4 weeks, with peak improvement at 8–12 weeks.
Formulations

It converts to active clindamycin upon contact with the skin, targeting bacterial colonies within the pilosebaceous unit. The emollient base helps maintain hydration without clogging pores.
Pairs well with
Benzoyl peroxide (essential combination per AAD guidelines—reduces resistance risk and enhances efficacy), adapalene or tretinoin (retinoid at night + clindamycin in the morning addresses acne from multiple pathways), niacinamide (supports barrier repair), and oil-free moisturizers. Avoid combining with erythromycin (cross-resistance) or harsh exfoliants that may increase sensitivity.
